This session provides an update on CHANGE (Canadian Health Advanced by Nutrition and Graded Exercise) Program, highlighting the experiences of two Ontario Family Health Teams that have implemented the program.

Speakers:


Rupinder Dhaliwal, is the Director of Operations, Metabolic Syndrome Canada, who is leading the expansion of this team-based diet and exercise intervention in primary care clinics across Canada. She provides an overview of the CHANGE program including program structure, the evidence supporting its efficacy and the partnerships created as the program is being expanded across multiple primary care settings in Canada.

 

Carolyn Tereszkowski and Jennifer MacTavish are both Registered Dietitians working at Ontario Family Health Teams who have implemented the program. Carolyn shares the steps involved in implementing the program at Loyalist FHT including establishing community partnerships, innovative methods to deliver the exercise component, the use of group sessions and program evaluation results. Jennifer works at Brockton and Area FHT and describes how they have implemented the program including the utilization of a Nurse Practitioner as the Primary Care Provider of the participants. Both Carolyn and Jennifer provide perspectives on how the CHANGE program can be implemented using the resources unique to a specific primary care setting.
