Older adults are at nutrition risk which is currently not being identified or supported to prevent malnutrition. This session will review nutrition risk screening in primary care and the development of consensus-based nutrition care pathways, including the new Malnutrition Prevention, Detection and Treatment Standard from the Health Standards Organization and a pathway to support the often precarious transition from hospital to home. This session will support the appraisal, analysis and integration of the Standard and pathways into hospital, primary care and community settings.
After this session, participants will be able to:
- Identify the evidence on outcomes of malnutrition and nutrition risk, including for community-living older adults;
- Appraise the strength of evidence for nutrition risk screening in primary care and development of these consensus-based nutrition care pathways and the Malnutrition Standard for Canadian Hospitals;
- Analyze the steps in the pathways and the various actions of the dietitian and other providers and how these are mirrored in the Standard; and
- Integrate components of the Standard and pathways into their hospital and primary care practice.
